Amity Point requires a referral for initial evaluations in all disciplines, please have your physician fax a referral to 563-551-7289. Click here to download our referral form to provide to your physician.

Out of Network Insurance and Private Pay
If you are out of network, superbills are available upon request. Superbills are invoices that have all the required information to submit an out of network claim. Although it's your responsibility to submit these out of network claims, we are available to assist and provide resources on the process. Contact our office if you have any questions.
Amity Point offers private payers a flat fee for all therapy disciplines to include initial evaluations and therapy sessions.
Therapy Evaluations, $120/evaluation
Therapy Sessions, $100/session
