To begin therapy at Amity Point, you can take the following steps:
Contact Us: Call us at 563-223-8404 or book an initial evaluation online. If you are unsure whether therapy services are needed, we also offer a free consultation to help determine the best course of action.
Obtain a Referral: If a referral from your physician is required (most insurance companies require), please provide us with the referral. Once we receive it, our team will promptly reach out to schedule an appointment.

While a referral from a doctor is not always required to begin therapy at Amity Point, most insurance companies do require one for coverage. We recommend checking with your insurance provider to confirm their specific requirements. If you need assistance obtaining a referral, please feel free to contact us. Once you have a referral, you or your physician can fax it to us at 563-551-7289.

If you are out of network, superbills are available upon request. Superbills are invoices that have all the required information to submit an out of network claim. Although it's your responsibility to submit these out of network claims, we are available to assist and provide resources on the process. Contact our office if you have any questions.
Amity Point offers private payers a flat fee for all therapy disciplines to include initial evaluations and therapy sessions.
Therapy Evaluations, $120/evaluation
Therapy Sessions, $100/session
